The July through September 2005 Highlighted Topics series examines the role of physical exercise in combating two related metabolic diseases, obesity and Type 2 diabetes. Skeletal muscle is by far the largest tissue mass sensitive to insulin-stimulated glucose uptake. Although much is known concerning insulin-and contraction-stimulated glucose uptake by muscle, the basic defect in insulin resistance that uncouples insulin binding to its receptor from GLUT4 translocation to the membrane is poorly understood. Likewise the relationship between abdominal obesity and insulin resistance is not clearly defined, but it is becoming increasingly apparent that hormone-like peptides are released from adipose tissue that can influence insulin sensitivity of skeletal muscle. Epidemiological and prospective studies clearly demonstrate that incorporation of regular exercise into our lifestyle enhances insulin sensitivity and has a protective effect against these inactivity related syndromes.
