Direct Measurement of Intrabronchial Pressure. Its Application to the Location of the Check-Valve Mechanism

A simple method is described for the direct registration of intrabronchial pressures in man. The method has been applied, in combination with the measurement of the airflow velocity, to the study of the location of the ‘check-valve’ mechanism which could be demonstrated to exist during voluntarily produced wheezing expiration in normal individuals. High intraluminal pressures between 70 and 80 cm H2O were found in the larger intrapulmonary cartilage supported bronchi, indicating that the main airflow resistance was located in the larger bronchi. These observations call for reinvestigation of the relative importance of the central and peripheral airway resistance in patients with asthma and emphysema.
